Jonathan Abenaim, D.M.D., a cosmetic dentist based in Hawthorne, N.J., tells Star George probably had work done because he grinds his teeth. &lt;br /&gt;&lt;br /&gt;"I can tell this because the edges on the bottom are not straight in the earlier photo," Dr. Abenaim tells Star. "When you grind your teeth, they get shorter. To restore the original look, the dentist has to lengthen teeth with porcelain veneers or crowns," says Dr. Abenaim. &lt;br /&gt;&lt;br /&gt;In addition to veneers, he thinks George had a "smile lift," a surgical procedure usually done with a laser that brings the gum up to expose more tooth. Though the cost of all this dental work likely set George back $30,000 to $35,000, Dr. Abenaim thinks it was well worth it. &lt;br /&gt;&lt;br /&gt;"It fills out his face more and makes him look younger," he says. &lt;br /&gt;&lt;br /&gt;What do you think of George's new smile? Bacteria live in the plaque and feed off the sugars and starches in food. As the bacteria digest the food, an acid is produced. This acid can eat away at the tooth enamel and lead to the formation of a hole, or cavity.&lt;br /&gt;&lt;br /&gt;Many of the foods we eat contain sugar and starch. Some foods are obviously high in sugar, like candy and cookies. However, sugar can be found in other “non-sweet” foods, like breads and crackers. Sugars are also added to some packaged foods, like peanut butter and condiments, to enhance taste.&lt;br /&gt;&lt;br /&gt;An important source of sugar in many American diets is soda. Researchers estimate Americans drink more than 50 gallons of soda each year. Diet sodas are not necessarily any better because soft drinks have a low pH, which can lead to erosion of tooth enamel. In fact, research shows cola drinks have ten times the erosive power of orange juice. Sports drinks and high-energy drinks also have a low pH and added sugars, and thus, may be as bad for the teeth as sodas. Another study found canned iced tea to be also very erosive. On the other hand, milk and cheese appears to protect the enamel and reduce the risk of tooth decay.&lt;br /&gt;&lt;br /&gt;Diet can also sometimes influence the health of gums. When plaque accumulates on the teeth, it can damage the tissue at the gum line and lead to the formation of tiny pockets between the teeth and the gums. As the condition progresses, the pockets get bigger. Eventually, the supporting structures for the teeth may be damaged and the tooth may fall out. Early stages of gum disease (called gingivitis) are characterized by tender, red, puffy gums that bleed easily. Periodontitis is the progressive form of gum disease, marked by tissue damage, weakening of structural support and eventual destruction of underlying bone and loss of teeth.&lt;br /&gt;&lt;br /&gt;Prevention &lt;br /&gt;Jonathan Abenaim, D.M.D., reminds patients the mouth is the entryway to the body and is impacted by everything we eat. He recommends everyone follow a healthy diet with plenty of fruits, vegetables, milk and dairy products.&lt;br /&gt;&lt;br /&gt;Enamel erosion from diet can often be prevented. The longer sugars stay on the teeth, the greater the risk of enamel loss. Try to avoid soda and limit the amount of sports drinks and acidic fruit juices you drink. If you must drink a sugary beverage, try to limit the amount of time the drink comes in contact with the teeth (i.e., don’t sip it all day long). Drinking sodas and juices through a straw also limits the amount of contact with the teeth. Don’t eat sticky, sugary foods because the sugars get stuck to the teeth for long periods of time.&lt;br /&gt;&lt;br /&gt;Abenaim also recommends rinsing the mouth with water after eating or drinking sugary beverages. Brush teeth with a fluoride toothpaste at least twice a day and floss daily.&lt;br /&gt;&lt;br /&gt;Saliva is a natural rinse for the bacteria that cause tooth decay. Toothaches and other dental inconveniences occur regardless of the fact that it may be 2 AM or that your dentist is away on a Caribbean vacation. Knowing what to do can prevent major discomfort.&lt;br /&gt;&lt;br /&gt;Toothaches can range from a distressing inconvenience to an intense, miserable experience. Severe, emergency dental pain may be as unrelenting as kidney stones or even labor contractions. Fortunately, the chance of an unforeseen dental crisis can usually be prevented by early detection and treatment of dental disease. When dental disaster does strike, (and it always seems to be at the most inopportune time) knowing how to alleviate the pain is invaluable information everyone can use.&lt;br /&gt;&lt;br /&gt;A toothache is any pain or soreness within or around a tooth, indicating inflammation and possible infection. Generally, a toothache occurs if tooth decay is very close to or has penetrated the pulp chamber that contains nerves and tiny blood vessels. Ideally, it’s best to undergo dental treatment at once.&lt;br /&gt;&lt;br /&gt;Dr. Jonathan Abenaim is a cosmetic and restorative dentist in New Jersey. Throughout his many years in practice, he has heeded the call of many a patient in dental distress. Dr. Abenaim has developed an arsenal of at-home tips to help patients with toothaches and other emergencies until they reach his dental chair. “If your toothache is caused by trapped food, you should rinse the area with warm water and swish it about. Floss between the teeth this should dislodge the food or it may loosen whatever is causing the pain.&lt;br /&gt;&lt;br /&gt;There are many over-the-counter (OTC) pain relievers to cure toothaches that you can choose from. Most common are paracetamol, aspirin, and acetaminophen. If you feel that a small swelling has occurred, you may take non-steroidal anti-inflammatory drugs (NSAID) for their inflammatory components, like ibuprofen, (Advil, Aleve) and mefenamic acid. However, those with a history of ulcers as well as pregnant women need a doctor's recommendation before taking NSAID and aspirin.&lt;br /&gt;&lt;br /&gt;“Rubbing aspirin on your gums to numb an aching tooth isn't a good idea. In fact, it'll do more harm than good,” says Dr. Abenaim. He adds, "Aspirin contains salicylic acid, which can burn and damage gum tissue. For general pain relief, it's a better idea to simply swallow the aspirin. Or stop by a pharmacy for pain-relieving gels like Anbesol or Orajel.” There is also something that a patient can buy called Dentemp. It’s over the counter and can be used to cement a temporary or permanent crown until the person gets to his/her dentist. It could also be used to caulk or fill a hole in case a filling breaks.&lt;br /&gt;&lt;br /&gt;Other home remedies for a toothache include rinsing your mouth with salt water or dabbing some clove oil directly on the bad tooth. Clove oil has bacteria-slaying properties, along with a remarkable numbing effect. "We've used clove oil in dentistry for years," Abenaim says. "Clove oil is responsible for that stereotypical dental office smell. Years ago, we would dab clove oil over a tooth before putting a filling in it, but now we have better ways of decreasing the sensitivity." For another home remedy, there is numbing power in cooled peppermint tea. Swish, then swallow if you like the flavor. "People can try putting some ice on the area, but the temperature of the ice could send them over the edge,” cautions Dr. Abenaim. “Most challenging are toothaches that stem from inside a tooth,” he adds. But in cases where a lost filling or a broken tooth is causing the pain, "caulking" the sore tooth with softened chewing gum can ease the pain - covering the sensitive area until it can be repaired.&lt;br /&gt;&lt;br /&gt;Dr. Abenaim also receives frequent complaints about canker sores. He suggests that people with frequent canker sores check to see if their toothpaste contains sodium lauryl sulfate, or SLS. SLS is a foaming agent that in some studies has been linked to the development of canker sores. "Toothpaste that doesn't contain SLS usually makes a big deal about it by saying 'SLS free' on its label," says Dr. Abenaim. An additional remedy for both canker sores and gum pain is tea tree oil. Tea tree oil should be applied directly to the infected site and can even be purchased in mouthwash form to soothe inflammation. Another preventive measure for canker sores is to avoid eating chips and other jagged foods that can cause tiny cuts and scrapes, which can be an entryway for a virus to set up shop in your mouth. Besides that, stay away from foods that trigger canker sores. Potential troublemakers include whole wheat, rye, barley, shellfish, pineapple, chocolate and tomatoes, as well as salty or citrus fruits. There is a remedy of 1/3 Mylanta, 1/3 Benedryl, and1/3 viscous Lidocaine. This will ease the discomfort of canker sores.&lt;br /&gt;&lt;br /&gt;"About 80 percent of our population deals with gum problems - from gingivitis all the way up to losing teeth," according to Dr. Abenaim. Ginseng can be used to alleviate the pain associated with irritated gum tissue. A ginseng tonic can be directly applied to the gum to promote circulation and speed healing. An additional home remedy suggestion is to use a wet tea bag for tooth or gum pain. Tea contains astringent tannins, which have the power to shrink swollen tissue and stop bleeding. No toothache will heal on its own. A toothache is a dental problem that needs to be treated as soon as possible.&lt;br /&gt;&lt;br /&gt;What should you do if you've knocked out a tooth - whether from a blow, a fall or accident?&lt;br /&gt;&lt;br /&gt;Shake off debris (rinsing or scrubbing could remove important periodontal ligament), place it in a container of milk or back in its socket and try to reach the dentist within 30 minutes. “Unfortunately, not all teeth can be saved,” cautions Dr. Abenaim. Alternative methods, such as a bridge or implants, may need to be considered."&lt;br /&gt;&lt;br /&gt;What should you do if you've broken a tooth?&lt;br /&gt;&lt;br /&gt;Again, if you've lost the broken piece, just get to the dentist. “If you have it, gently shake off surface dirt and definitely keep it,” says Dr. Abenaim. "Even if the piece can't be bonded back to the original tooth, it can be used to help re-create the look of that tooth."&lt;br /&gt;&lt;br /&gt;If something (food, other foreign matter) is wedged between your teeth, should you try to dislodge it?&lt;br /&gt;&lt;br /&gt;Gentle flossing or brushing is fine, but if the object does not respond to that treatment and the surrounding gum begins to swell, make an appointment with your dentist. A bit of broken filling or chipped enamel can throw your bite off and make you panic. Popcorn husks are one of the worst problems and occasionally need to be removed by a dentist.&lt;br /&gt;&lt;br /&gt;What should you do if you lose a filling or crown?&lt;br /&gt;&lt;br /&gt;If you lose a filling, it's not worth saving. However, just cover the hole with temporary material, do not try to put the old filling back in the tooth. If you lose a crown, you'll want to try and salvage it (you can use Temparin or Dentemp, available in pharmacies, to put the crown back in place until you can reach a dentist). “In either case, visit the dentist as soon as possible,” says Dr. Abenaim. "Many people don't realize that when a filling falls out, there's a reason for that, whether it's a cracked tooth or more severe. I can't tell you how many times before we do a smile makeover our patient shows us a picture of a model or a celebrity and says I want to have his or her teeth. Unfortunately many celebrities have fake teeth and they look fake. Beautiful looking veneers don't always have to look white and monochromatic. Teeth are several diffrent shades of white and have different angles to reflect light and show a beautiful apperance. So lets look at my favorite celebrity smile. In less that two sessions we can transform her smile.&lt;br /&gt;&lt;br /&gt; We all love tom cruise and many of us can say that he has had a long journey with his teeth. He's had braces, he's had veneers/ crowns I thikn twice. Now if we look closely, yes he may have white teeth/restorations but his gums are in bad shape. We must always keep in mind that flossing and regular dental appointmens will really maintain thouse crowns/veneers or else gum recession may occur and if you have a smile like tom it may be a cosmetic problem for you. Implants are made of Medical grade Titanium or Titanium Alloy. Your dentist will purchase the implants in sterile packaging and place them in a small hole in the bone which he or she has prepared as part of the implant placement procedure. Each office is involved with implant dentistry. Implants involve two phases of treatment, placement of the implant(s) and placement of the crown(s) or restoration(s). Some dentists feel they can offer better solutions for you by using the different expertise of other team members, e.g. performing the surgery or the restoration. Other practitioners feel that performing both the surgery and the restoration within one office is optimal for many patients.&lt;br /&gt;&lt;strong&gt;What exactly are dental implants and what can they do for me?&lt;/strong&gt;&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;Implants are substitutes for teeth and they are today's best alternative to your natural teeth. They offer you a permanent or secure solution for replacing one or more teeth. They are made of biocompatible materials, just the same as hip implants or similar orthopedic devices, and function as anchors or support for traditional forms of dentistry, such as crowns, bridges or dentures. Many of our patients will tell you that implants have changed not only their smiles, their overall appearance but also even their lives! There are numerous other reasons to choose dental implants:&lt;br /&gt;Esthetically, they support teeth that look like real teeth. Functionally, dental implants feel and act like real teeth.&lt;br /&gt;With implants, you can eat and chew again without pain or irritation. Foods that were forbidden are now back in the diet.&lt;br /&gt;Implants eliminate the need for distasteful adhesives. There is no longer a need to use "glue" when your prosthesis is anchored to implants.&lt;br /&gt;Implants can actually improve the taste of food. With less plastic covering the roof of your mouth, you can enjoy natural flavors and sensations again.&lt;br /&gt;Lastly, implants can help maintain your bone structure and support your facial tissues. They can reduce or eliminate bone atrophy, which causes "shrinkage" or facial cosmetic changes.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Are implants successful? &lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;br /&gt;&lt;/strong&gt;Implants, as we know them today, have been in existence for at least twenty-five years. For the last ten years, however, success rates at many treatment centers are consistently over 95% with proper personal and professional care. Few forms of medical, orthopedic or dental treatment have such high success rates.&lt;br /&gt;What is the procedure like?&lt;br /&gt;There are actually two phases to implant dentistry:&lt;br /&gt;Phase 1:&lt;br /&gt;Using very accurate surgical techniques, an incision is made in the gum tissues and implants are placed into dimentionally controlled sites (depth and width) in the jawbone. The gum tissues are then closed and the healing phase begins. This may take anywhere from 3-6 months to ensure a strong base.&lt;br /&gt;Phase 2:&lt;br /&gt;Creating and fixing the new tooth or teeth to the implant structure constitutes the second and final phase. Implants can replace a single tooth, several teeth or your dentures.&lt;br /&gt;&lt;br /&gt;&lt;/strong&gt;&lt;strong&gt;Are they expensive? &lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;br /&gt;&lt;/strong&gt;Initially, you might feel dental implants are expensive. However, they represent a more permanent and natural solution than dentures and partials. Thus, there is a greater initial investment. They should last for years, particularly if you have an "ideal" case. Independent surveys of implant patients show that they felt the investment was worth every penny and would do it again!&lt;br /&gt;&lt;strong&gt;How much do they cost? &lt;/strong&gt;&lt;br /&gt;&lt;/strong&gt;&lt;strong&gt;&lt;br /&gt;&lt;/strong&gt;Each case is different. Cost is a function of your particular needs. After a complete examination, your doctor will more specifically determine your dental needs and the best treatment alternative(s) for you. At that point, you can specifically address the question of cost and be helped with financial arrangements.&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;Does it hurt?&lt;/strong&gt;&lt;br /&gt;Usually, discomfort is not a problem. In fact, many have said that having a tooth pulled was more painful! For the placement of an implant(s), your doctor will use one of many anesthetic regimens so there won't be any discomfort. When you leave the office, you will be given appropriate medications. We urge you to ask other patients how they felt after implant therapy. Your doctor(s) and their office teams are available at anytime to make sure that you are comfortable.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;What benefits can I really expect?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;Eat whatever you like! Implants improve your health with the ability to chew a wider variety of foods.&lt;br /&gt;Smile with confidence. Discolored or missing teeth ruin your appearance. Implant supported restorations help increase your self-esteem. People will approach you more readily.&lt;br /&gt;Express yourself clearly or simply be understood. Implants increase the retention and stability of your teeth.&lt;br /&gt;Improve your external facial structure and decrease the likelihood of continued bone loss. Your jaw and teeth will smile together.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;How long will it take? &lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;br /&gt;&lt;/strong&gt;Recent advances by implant manufacturers are reducing the time for initial implant procedures. Treatment for the simple cases could take anywhere from 4-8 months, based on where the implants are placed and the type of restoration you desire. Do not compromise because your implants and the new teeth that they support should last for years.&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;Will I be without teeth?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;You will never be without teeth. Your doctor will make sure that in the interim period, you look and feel very natural.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Am I too old?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;Implant patients can be in their early teens or be mature adults in their seventies and eighties. Good health and adequate bone are the most critical factors in evaluating implant candidates.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Is one implant system better than the others?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;No. There are many FDA approved implant systems available to your doctor.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;What will it feel like when it is done?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;Once the entire procedure is complete and the final restorations placed, they will actually feel or&lt;br /&gt;function like your natural teeth!&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;How do I take care of my implants?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;Normal routine home care and professional cleaning visits are required. It is important to know that every 3-4 months, you will need to have a routine cleaning and re-care treatment and evaluation. An individualized program will be designed for you to optimally care for your new teeth.&lt;br /&gt;&lt;strong&gt;Are there alternatives?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;Your doctor should describe all reasonable alternatives. Then you will make a decision together as to what is best for you.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;When can I get started?&lt;br /&gt;&lt;/strong&gt;As soon as your doctor has all the necessary information and team members in place to give you excellent short term as well as life long care.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Can any licensed dentist(s) legally perform implant surgery or restoration? &lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;br /&gt;&lt;/strong&gt;Yes. However, education and experience are critical! Ask your dentist or dental specialist questions about his or her training in implant therapy and in-office implant experience. Over 30 years ago, Dr. P. I. Brånemark put together a technique for placing and restoring dental implants that proved to be far superior to anything that had come before. Dr. Brånemark’s use of Titanium for the construction of the implant proved to be a boon to the profession and the patients who were missing teeth. The procedure was almost 100% successful and lead to the development of major dental implant markets throughout the world providing thousands of patients with “new teeth”. There was one problem, however, in that it took literally months to get these new teeth and many patients and dentists did not want to wait this long. Researchers and clinicians began to study methods of speeding up the process and one of the major results of this research lead to “teeth in an Hour”. Teeth in an Hour is a method utilizing radiographic 3-dimensional scans to make a totally accurate representation of the patients jaw and then constructing a mechanical template on this jaw which allows the dentist to place implants with extreme precision. As a matter of fact, the precision is so good that you can actually make up the final restoration (the new teeth) on the model and transfer everything exactly to the patient’s real jaw. Using the template in the patient’s mouth, the implants are precisely placed and then the new teeth are attached allowing the patient to leave within and hour or two or three with a complete set of teeth ready for action.&lt;br /&gt;    Benefits:&lt;br /&gt;Of course, you get your teeth right away instead of waiting 3 to 6 months or more. You also benefit from faster healing with the implant surgery and implant placement is more precise than if the dentist did it by hand.&lt;br /&gt;    Problem Areas:&lt;br /&gt;This type of treatment is much more expensive than the “old fashioned” way of placing implants and waiting until they are ready for the new teeth. If it is not done absolutely correctly, the new teeth may not fit and there may be a significant delay in getting the teeth. My practice does not utilize amalgam fillings. The reason is not because they are hazardous, or they are inferior materials, but because my patients just do not want silver fillings in their mouths for esthetic reasons. Here is an article written by Dr. Gordon Christensen, one of the guru's in dentistry. Inquiries are welcome.&lt;br /&gt;Happy reading&lt;br /&gt;Dr. Jonathan.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;The Use of Mercury Containing Restorations (Fillings) in Dentistry&lt;br /&gt;Gordon J. Christensen, DDS, MSD, Ph.D.&lt;br /&gt;&lt;/strong&gt;"Dental amalgam containing roughly 50% mercury has been used in dentistry for well over 100 years. Almost all health organizations around the world support the continued use of silver amalgam. However, there is continuing controversy about the desirability of having mercury containing restorations in the mouth, because of the minute amounts of mercury that are released from these restorations. It is highly unlikely that such small concentrations of mercury can or do harm the typical human. However, there is a remote chance that persons who have immunocompromised systems could have some negative influence from this mercury presence or the presence of other metals.&lt;br /&gt;Around the world, silver amalgam is in various states of challenge. In Germany, silver amalgam has been eliminated from use. In Sweden, it is planned that silver amalgam use will be discontinued next year. In numerous other countries, silver amalgam is being taught to a less degree than in the past in the dental schools because of alleged contamination of sewer lines and other biologic hazards. Nevertheless, almost every health organization throughout the world supports the continued use of silver amalgam. Many people do not want silver amalgam in their mouths because of its unesthetic appearance, and we are finding that the change from silver fillings to white fillings has been related more to persons desiring esthetic dentitions than it has been to alleged biologic hazards.&lt;br /&gt;There are many elements used in the mouth that are potentially allergenic. Among them is mercury. However, far more potentially allergenic is the element nickel, which is used in many dental restorations. Additionally, there are many other elements used in the mouth that could cause allergic reactions in people, manifested by red gums, a burning taste in the mouth, and signs that are evident only to dentists.&lt;br /&gt;The other potentially negative aspect of silver amalgam is the toxicity question. This is still unanswered, and it is likely that it will remain unanswered because of the confusion that is currently present in the scientific area concerning the alleged toxicity of silver amalgam.&lt;br /&gt;There are many new materials that are available for restoration of teeth excluding silver amalgam. Among them are composite resin white fillings that are used for small restorations in teeth, and new polyglass crowns that are available to replace the typical porcelain-fused-to-metal crowns that have been the mainstay for teeth that are very broken down. It is likely that continued research will produce materials that will eventually eliminate the use of metal in the human mouth.&lt;br /&gt;In summary, silver amalgam continues to be used as the major restorative material in dentistry throughout the world. Its alleged allergenicity and toxicity are argued in the most scientific of organizations, but conclusive evidence concerning the need to eliminate this restorative material from common usage is still highly controversial. If patients desire to have tooth colored restorations, they are available today, and most of these have little or no known toxicity or allergenicity. Patients who desire to have elimination of silver amalgam from their mouths for esthetic reasons should contact their dentist to see if this is available from the practices in which they are involved.
